In April of 2024 Mark BUFF observed a crew member placing his hand inside a rebar cage to run test wire while lowering the rebar cage into pile hole. This could lead to the employee getting caught into the rebar cage and be pulled into the installation location or hand and arm injury. Mark alerted the crew to stop the task. Shared his observation with the crew and a plan was built to use rope to install the wire before setting cage into test pile. |
In March of 2024 Dwayne JOHNSONwas assigned fire watch duties, while using torch to cut sheet piles, green Dwayne noticed the acetylene hose had caught fire where the hose makes connection to the torch. He used his air horn to alert the rest of the crew. Once realized, both gauges were closed off. Once flame was out, the hose and gauges were inspected for damage. No damage was found but during the inspection it was identified that the fittings were not properly tightened. |
march 2024 - During the process of installing a pair of sheet pile, Clay heard a popping sound which is uncommon for sheet pile. Clay searched for the source of the sound and while investigating inspected the sheet pile being installed, discovering at the top of the sheet pile where the vibo-hammer jaws attached to the sheet pile, the portion of the sheet pile inside the jaws had cracked, separating from the sheet pile. If this was not discovered would have created a dropped/falling object incident.
Once discovered, Clay alerted the crew of the finding. Work was stopped and crew made to clear the area around the vibro-hammer. Once clear, the crane operator repositioned the vibro-hammer to allow for the broken piece of sheet pile to brought to the ground. Once on the ground the sheet pile piece was found to be 8"x6' in size. In addition to receiving the Good Catch Coin for BU15, Clay was awarded 1st Place for the Corporate Good Catch Award Program as well. |
In February of 2024 Eric Lane identified while using a crane with swinging leads, the hook on the headache ball holding the leads up didn't have a keeper pin in the hook. If the rigging was to shift there would be potential for the leads to slide out from the headache ball's hook. Eric stopped the work and had the operator lower the leads to the ground. Once the load was secured informed my crew of the observation and got a keeper pin to put in to keep the leads from unrigging. |
In February of 2024 David Kirksey While standing up leads, saw block sheave pin coming out and about to fall out while standing up a set of lead. If fallen completely out could have resulted in additional parts of the block falling below onto ground crew. David alerted the crew by sounding the crane's horn. I stopped all activity and directed the crew to get back and stay clear. Once area was clear I lowered block and had the crew installed a new safety pin in bar. |
In January of 2024 Jaron Edgerson noticed while setting up the leads and hammer noticed that the cable was ran over the sheave pin. If this was not found the hazard could have damaged the crane cable and lead to a dropped load incident. Jaron alerted the crew as soon as he identified the issue, work was stopped, and crew pulled the cable out and reran it the proper way. |
In October of 2023 Delicio Raymond noticed while raising the hammer to bail a lot of shaking with hammer cable. Delicio got with the crane operator to lower hammer to inspect, noticed sheave pin wasn't all the way in because the cotter key had sheared off.
